- Email the completed DNA3 and DNA4 forms to the Simbra Office.
• DNA3 in Excel format.
• DNA4 in PDF format.
• No pictures, scanned or handwritten copies of the DNA3 form will be processed. - The office will generate a batch number on the system and email the breeder a lab acknowledgement letter along and lab request form.
• The breeder should inform the office should there be any errors in this document. - The breeder must send the samples to the following address at their own expense:

- Clearly mark the parcel with attention for Jody.
- Send the courier/delivery information with a tracking number to the office.
- The office will send the samples to Absolute Genetics where they will then be sent to the lab.
- When the results are received from Neogen, it will be loaded on the system.
- The office will send the breeder a genetic conditions report (where applicable) and inform the breeder should there be any samples that failed genotyping.
- Note: All courier costs are to be carried by the breeder.

• Lab request form. - When the samples have been analysed, Unistel will send the breeder a batch details report.
- The breeder must send this report to the office where the genotyping details will be captured on the system.
• In the case of SNP genotyping, the office will ask Unistel for the raw data using the batch number in the batch details report. Once the raw data is received, it will be loaded on the system.
*Unistel also have complimentary sending options in which Unistel bears the cost of the parcel if it is delivered to specific drop off points. Please contact the office for more information.
